CMS is Ahead of Schedule... Are You?

Law,Health,Business News It’s only 2015 and CMS has already reached 42% of their 50% goal to tie traditional Medicare payments to quality or value by 2018. Humana has implemented a similar strategy and set a 50% goal by 2018 and Aetna further has upped the ante by striving to tie a whopping 75% of Medicare Advantage members to ACOs by 2017! It’s clear the tide is moving from Fee for Service to Value Based Payments, and the goals of value based programs have been spelled out for us: Better Health for the Population, Better Care for Individuals, and Lower Cost through improvement. What does that mean? It means that your practice must now learn to bend both the quality AND cost curve. But where to start? Before you even consider entering in to an agreement, 1.Metrics for both the quality and cost curve must be established; 2.A baseline assessment of your practice for applicable quality measures must be completed; and 3.A plan for improvement should be drafted so that your impact has a path. Only once you are prepared can you begin to devise contracting strategies that capture the strength of your practice. To learn more about healthcare reimbursement issues visit our blog HERE

7 Tips for Taking Care of Your Braces

Health According to Humana, more than 4 million people in the United States wear braces. They are a proven way to achieve that perfect smile you’re looking for, and they work well on adults and children alike. However, braces need special care in order to keep your mouth as healthy as possible. Watch What You Eat One of the first things you will learn at your Southlake orthodontics office visit is that there are certain foods you shouldn’t eat while wearing braces. Hard or chewy foods can damage your braces or even pop your brackets off your teeth. Chewy foods can also get stuck in your braces, making it difficult to keep your teeth clean. Some foods to avoid include the following: •Gum •Chewy candy (Starbursts, Twizzlers, Skittles, etc.) •Caramel •Nuts •Pretzels Cut Your Food You should also avoid foods you have to sink your teeth into, such as apples or corn on the cob. This also goes for foods like burgers, pizza, and sandwiches. Like hard foods, these can put too much pressure on your braces. Instead, cut your food into bite-sized pieces so you can chew it with your stronger back teeth. Stop Chewing Your Nails Chewing on hard objects such as your pencils or fingernails can also damage your braces. Unfortunately, this is a habit that is ingrained in some people and can be difficult to break. Ask a friend or family member to remind you if they see you starting to slip back into old habits. You could also try sucking on sugar-free hard candies to give your mouth something else to do--as long as they aren’t too sticky. Brush Thoroughly Brushing your whole mouth thoroughly is especially important when you have braces. Although braces can’t develop cavities like your teeth can, they are still able to harbor bacteria. These bacteria can spread to the rest of your mouth and cause cavities, gum disease, and other problems. Not only can these conditions be painful, they can keep your mouth from healing, making your teeth less responsive to your braces. When you have braces, it’s important to brush your teeth at least twice a day. To keep them as clean as possible, brush them after every meal. Floss More Effectively Flossing is an important part of keeping your mouth clean, cutting down on the amount of bacteria that's able to hide between your teeth. However, it becomes extremely difficult when wearing braces. One effective way of flossing is to use a Waterpik. This uses a stream of water to floss, so you can easily clean between your teeth, even when you’re wearing braces. It’s also good at getting hard-to-reach spots at the back of your mouth and below your gum line. Plenty of Water Drinking water throughout the day is important for many reasons, but it’s one way to keep your braces in good shape. Drinking water helps loosen food particles stuck in the braces and washes away bacteria and plaque that have started to form. This keeps your braces clean and your mouth as healthy as possible. Protect Your Mouth Do your best to avoid impacts to your face while wearing braces. Not only is it painful, the impact may damage your braces. Be cautious in everyday situations that might result in injury. Anyone involved in sports should wear appropriate mouth protection, especially if you play a contact sport. Keep in mind that most over-the-counter mouth guards do not work well with braces. Ask your southlake orthodontics office if they can provide you with a mouth guard that is designed to fit over your braces. UnitedHealth Housekeeping Spurs Settlement Trend for Providers

Law,Business,Health This summer brought about insurer merger acquisitions of Cigna by Anthem and Humana by Aetna, which leaves UnitedHealth as the only remaining major insurer not to make such an announcement. However, the UnitedHealth Group Inc. very recent settlement of a suit over 15 years old and another settlement of a suit that was over 6 years ago is a typical business effort to clean up their books in preparation for a huge transaction of this sort. Here’s the backstory: In May 2015, UnitedHealth Group agreed to pay $11.5 million to settle lawsuits brought by the state medical societies and individual doctors. The dispute arose over how UnitedHealth pays and codes claims. Specifically, the lawsuit claimed that UnitedHealth was systematically denying payments to doctors for medically necessary claims solely to meet internal financial goals. It was alleged that UnitedHealth programmed its software to automatically downcode claims and penalize doctors whose charges weren’t within a certain range of treatment costs. When the lawsuit was originally filed over fifteen years ago, it represented medical societies in 19 states. At the time of settlement, only four states remained and included Connecticut, New York, North Carolina, and Tennessee. As part of the settlement, UnitedHealth agreed to spend at least $9 million to enhance the insurer’s website where providers file claims. The settlement also required money to be paid by the insurer to the Connecticut State Medical Society Physician’s Health and Education Fund and will be spent on educational programs for members regarding: medical coding, medical review audits, appeals, submitting and processing electronic claims. Physician Advisory Councils will be created in Connecticut, New York and Tennessee that will help doctors and providers resolve issue or problems with UnitedHealth. While settling with the above class of plaintiffs in May 2015, UnitedHealth then settled with another class of plaintiffs that were comprised of ambulatory surgery centers (ASC’s) according to the press release on September 10, 2015. The class action was filed over six years ago by a group of out of network California Ambulatory Surgery Centers. The complaint alleged that UnitedHealth improperly calculated the reasonable and customary amounts for out of network ASC’s resulting in overpayments. According to the press release, UnitedHealth has agreed to pay $9.5 million and the parties estimate that there are approximately 250 ASC’s that could qualify to participate in the settlement. The allegations outlined in each of these suits (suddenly settled within 4 months of each other) run in parallel to the developing trends in southern markets. Both out of network and in network providers in Florida are increasingly faced with denials and overpayment demands from Cigna (currently involved in a major merger with Anthem). Providers who haven’t already started documenting all of their payer communication (i.e. email, fax, in person meetings and phone calls), compiling claims that have been impacted and calculating an aggregate of the financial impact will be inadequately equipped to combat the expanding payer powder keg.
